[Madlug] qmail authentication

Madlugers,

I'd like to install Krysztof Dabrowski's (and Eric M.
Johnston) qmail-smtpd-auth patch on a qmail server
(also running SA, vpopmail, qscanner).  I'm having
trouble finding good documentation on install and
set-up.  Can anyone point the way?
